Information We Collect
In order to better provide you with our services, the Site collects two types of information about its visitors: Personally Identifiable Information and Non Personally Identifiable Information.
Personally Identifiable Information: “Personally Identifiable Information” refers to information that lets us know the specifics of who you are and can be used to identify, contact or locate you. Personally Identifiable Information is also requested when you correspond with us, or otherwise volunteer information, for instance, through the use of other services on the Site. Personally Identifiable Information can include your name, mailing address, credit card number, telephone number, email address, userid, password, and other identification and contact information.
Non Personally Identifiable Information: “Non Personally Identifiable Information” refers to information that does not identify a specific individual by itself or in combination with other information. We gather certain information about you based upon what you view on the Site in several ways. Our primary purpose in collecting personal information is to provide you with a finely customized and tailored experience. This allows us to provide services and features that most likely meet your needs. We only collect personal information about you that we consider necessary for achieving this purpose.
In general, you can browse the Site without telling us who you are or revealing any personal information about yourself. Once we receive your personal information, you are not anonymous to us. If you choose to use our services, we may require you to provide contact and identity information, billing information, shipping information and other personal information as indicated on the forms throughout the Site. Where possible, we indicate which fields are required and which fields are optional. You always have the option to not provide information by choosing not to use a particular service or feature, but certain portions of the Site and our services will be unavailable to you.
We may access certain information based upon your behavior on the Site. This information may include the URL that you just came from (whether this URL is on the Site or not), which URL you next go to (whether this URL is on the Site or not), your computer browser information, and your IP address.
If you send us personal correspondence, such as emails or letters, or if other users or third parties send us correspondence about your activities or postings on the Site, we may collect such information into a file specific to you.
If you establish a credit account with us to pay the fees we charge, we collect some additional information, such as a billing address, a credit card number and a credit card expiration date and tracking information from checks or money orders.
If you choose to post messages in our message areas or leave feedback for other users, we will collect that information you provide to us. We retain this information as necessary to provide customer support and troubleshoot problems as permitted by law.
Our Use of Your Information
We will disclose your personally identifiable information to third parties for tracking purposes, to charge your credit card (or process your check or money order, as appropriate), fill your order, improve the functionality of the Site, perform statistical and data analyses, deliver your order to you from us. For example, we must release your credit card number to confirm payment; and release your mailing address information to the delivery service to deliver products and services that you ordered.
We also use personal information to provide you our services, including the Starting Point Assessment results and reports based on those results. We will take the information you provide us, which includes your responses to the Starting Point Assessment, and prepare proprietary reports about you, which include the results of our analysis on issues such as your learning keys, strategy for efficiency and effectiveness, energy maps,performance cycle, and other key analysis of you, including an assessment of how you learn.
Our Disclosure of Your Information
Other than the third parties stated above, 5 Dynamics does not release your personal information, including email addresses, to any other parties such as third party mailing lists. Your email address will only be used for internal communication within the system and for receiving emails pertaining to the 5 Dynamics application.
Our services permit certain other people to access your tests and your personal information. As an individual, you may have the ability to see your own Starting Point Assessment results, see Wizards assigned by your Administrator or Manager and you can belong to a Group, which is overseen by a Coach-level user who can communicate with you. Coach-level users see their own Starting Point Assessment results,as well as the results for all individuals in their Group. Coach-level users have a home page which indicates when individuals have completed certain assignments and this page will disclose information about you to that user. Coach-level users also have access to reports describing the dynamics of 2 individuals working together, may view reports describing the dynamics of the Coach-level user working with an individual and also have certain administrative privileges that permit them to create new accounts and create and send email invitations using customizable email templates. Additionally, there are Superusers, also known as administrators, who have additional access rights and oversee several Coach-level users and their Groups of individuals. Superusers see all the accounts of Coach-level users and individuals in their ‘MasterGroup’, can add accounts, clone or move individuals between Groups, see scores and create emails and email templates. Coach-level users and Superusers will have access to your Personally Identifiable Information, as well as your test results,among other information, and by using our services and the Site, you are agreeing to give these individuals access to this information.
Control of Your Password
You are responsible for all actions taken with your UserID and password, including fees charged to your account. Therefore, you must not disclose your password to any third parties. If you lose control of your password, you may lose substantial control over your personal information. Therefore, if your password has been compromised for any reason, you should immediately change your password as detailed on the Site.
Your information is stored on 5 Dynamics’ servers located in the United States. By using the Site and our services, you consent to have your data, including your Personally Identifiable Information, processed in the United States.
We have implemented reasonable procedural and technical safeguards to protect your personal information against loss or theft as well as unauthorized access and disclosure to protect your privacy, including encryption and “firewalls.” However, “perfect security” does not exist on the Internet and we cannot guarantee the security of any information that is disclosed on-line.
We do not knowingly collect personal information from children under the age of 13. Children are not permitted to use the Site or services, and we request that children under the age of 13 not submit any personal information to the Site. Since information regarding children under the age of 13 is not collected, we do not knowingly distribute personal information regarding children under the age of 13.
